Childcare Need Form
If you are in need of childcare for an event that you are hosting, please fill out these details so the Kids Min Team can prepare accordingly. Unless specified, this will be basic childcare for the event.
If there are 15 or less kids, please fill this form out at least 2 weeks in advance. If there are 15 or more kids, please fill out this form 4 or more weeks in advance. Depending on the scope of the event, we may ask that you provide 2 - 4 additional childcare workers within your zone to help out with childcare if more than 50 kids will be in attendance.
