All Peoples Staff Incident Report

This incident report is to be filled out in order to record details of an unusual event that occurs at our church facilities or any All Peoples Church ministry related event. The recorded information may be useful in the future when dealing with liability issues stemming from the incident or helpful in maintaining proper use of facility space or continuing the ministry related event. What is shared will be kept private to parties this does not concern. You will be contacted by a member of the Senior Leadership Team to confirm this report.
